Invitation Letter

To date the majority of patient referrals in Greater Manchester have been made via invitation letter.

Your locality lead for the Healthier You service will have been in contact with your practice to ask you to run a search of your clinical system, identify eligible patients and organise for invitation letters to be sent out.

The invitation letter template was installed at your practice by the GMSS Data Quality Team named ‘NDPP Healthier You Invitation Letter’. This will be saved where documents are usually saved in your clinical system. If you cannot locate the form, please email gmss.dataqualitygm@nhs.net including your practice name and clinical system in use so that the correct version can be sent to you to import.
