Patient Eligibility Criteria
In order to be eligible to join the service patients must meet the following eligibility criteria:
- 18 years old and over
- Registered with a GP Practice in Greater Manchester
- HbA1c between 42-47 mmol/mol (6.0%-6.4%) or Fasting Plasma Glucose between 5.5-6.9 mmols/l within the last 24 months
- Women with a previous diagnosis of GDM with a normoglycemic blood result within 24 months
- Not pregnant
- Not on end of life pathway
