NHS Type 2 Diabetes Path to Remission Programme (formerly the NHS Low Calorie Diet Programme) Provider Transition FAQs for Service Users
From 1st June 2023, the NHS Low Calorie Diet Programme will relaunch under a new name and provider. The programme will now be called the NHS Type 2 Diabetes Path to Remission Programme and Momenta Newcastle (Momenta) will replace Xyla Health & Wellbeing (Xyla) as the provider of the service in Greater Manchester.
We have answered some frequently asked questions (FAQs) about this change and what is means for service users.

Q. What is the NHS Type 2 Diabetes Path to Remission Programme?
The programme provides life changing support for people who are overweight and living with type 2 diabetes. The programme provides a low calorie diet treatment for people to improve their diabetes control, reduce diabetes-related medication and in some cases, put their type 2 diabetes into remission.

Q. How is the NHS Type 2 Diabetes Path to Remission Programme delivered?
On the programme people are provided with low calorie, total diet replacement (TDR) products including soups and shakes consisting of 800 to 900 kilocalories a day for 12 weeks. During this time, participants will replace all normal meals with these products.
Alongside this, a 12-month period of behavioural change support is provided to enable successful completion of the TDR, food re-introduction and subsequent weight maintenance phases of the programme. Momenta’s specially trained coaches will provide participants with the help and advice they need throughout every stage of the programme.
People will have a choice of meeting their coach in-person or through digital delivery formats.
Q. Why is the programme moving to Momenta?
The programme is funded by NHS England nationally. As the contract with Xyla was due to come to an end, a process was undertaken to decide who would run the service from June 2023. This resulted In Momenta winning the contract to provide the service from 1st June for the next 2 years.
Q. Who currently runs the NHS Type 2 Diabetes Path to Remission Programme?
In Greater Manchester, the programme has been provided by Xyla under the NHS Low Calorie Diet Programme name. From 1st June 2023 they will no longer run the service and will stop accepting new referrals from Wednesday 31st May 2023. At that point GPs will make referrals to Momenta who will be providing the service.
Q. I’ve been referred to the NHS Low Calorie Diet Programme but haven’t yet started the programme, what will happen to me?
If you’ve already been referred, Xyla will try and get you started on the programme before the end of July. If you do start the programme before the end of July you will continue to be supported by Xyla until you finish the programme.
If you don’t start the programme by the end of July, Xyla will contact you to ask for your permission to share your information with Momenta so that they can contact you to start the programme with them.
You will receive a letter from Xyla about this. If you do not give consent to your information being shared with Momenta you will be discharged from the service. This would mean your GP would have to re refer you into the programme with Momenta.
Q. I have started the NHS Low Calorie Diet Programme but I won’t have finished by end of July, what will happen to me?
If you have started the NHS Low Calorie Diet Programme with Xyla before the end of July, don’t worry you won’t need to do anything. You will continue to be supported by Xyla until you complete the programme.
Q. Why is this change happening?
The current contract was coming to an end and the decision was made, following due process, to award the contract to Momenta. The NHS in Greater Manchester is working closely with Xyla and Momenta to ensure the service transfers safely and that people already on the programme continue to be supported. NHS England took the decision to change the name nationally following feedback from patient focus groups who feel the new name reflects the programme better.
Q. Will the service change when it moves to Momenta?
The service will continue to work much the same as it does now. 12 weeks of low calorie, total diet replacement will be provided to participants alongside ongoing support by a specially trained coach. Q. How do I access the NHS Type 2 Diabetes Path to Remission Programme?
If you have been diagnosed with Type 2 diabetes in the past 6 years and meet the eligibility criteria, you can access the Type 2 Diabetes Path to Remission through a referral from your GP practice.
