Integrated Diabetes Service

Stockport Diabetes Service is a fully integrated team between hospital and community led by three Diabetes consultants at Stepping Hill (Stockport NHS Foundation Trust).

Within the diabetes service, there is a dedicated team of specialist nurses, a specialist midwife, specialist dieticians, high-risk podiatrists and a team of diabetes educators that work across acute and community settings. The whole team provides training and support for staff and patients with type 1 and type 2 diabetes.

Nutrition and Dietetic Service

Diabetes X-PERT Programme is an award-winning six-week group education programme for Type 2 diabetes. It will help you understand more about diabetes and how you can manage it through lifestyle changes. Suitable for anyone newly diagnosed and those who have had diabetes for a while but want to learn more.

Each programme is run by two experienced health professionals with 2½ hour sessions each week for 6 weeks. Sessions are held on different days in various venues around Stockport. You will need to be registered with a Stockport GP to attend.

Also available: SOCCER – Stockport Optimising Carbohydrate Counting, Education & Results – a four-day course for Type 1 diabetes covering carbohydrate counting and insulin adjustment.

Podiatry

Podiatrists are healthcare professionals trained to diagnose and treat abnormal conditions of the feet and lower limbs. They also prevent and correct deformity, keep people mobile and active, relieve pain and treat infections.

Podiatry Service is aimed at people with 'at risk' feet, including those with poor circulation, loss of feeling, or other conditions placing them at risk of ulcerations or infections. We offer diabetic foot screening and assessment including vascular and neurological assessments.

Appointments: Available Monday–Friday, 8:00 am–5:00 pm at 14 clinic locations across Stockport. Contact 0161 204 4657.

Community Diabetes Services

Community Diabetes specialist nursing service comprises nurses offering education, support and advice to patients with type 1 and type 2 diabetes. The nursing service compliments the consultant service and together they offer holistic assessment and management.

Six Core Functions:

  • Pregnancy – Type 1/Type 2 and Gestational diabetes

  • Young persons service including transition from paediatrics

  • Insulin pump initiation and management

  • Alternative injectable therapies for complex patients

  • Insulin therapy and management for complex patients

  • Patient/staff education

Support Groups

Stockport Tadpole Group – Provides help and support to children, their families and carers living with diabetes in the UK.

Wellbeing and Self-Care Service

Wellbeing and Self Care Service provides community link, self-care and engagement support. Practice-based Self Care Coordinators offer one-to-one and group work to support people diagnosed with, or at risk of, long-term conditions.

Support available for:

  • Long-term conditions (e.g. Hypertension, Anxiety, Depression, Asthma, Obesity, Diabetes)

  • Lifestyle issues (e.g. diet, physical activity, weight management)

  • Money issues, relationships, housing, employment

  • Loneliness/social isolation

  • Caring issues

The Prevention Alliance (TPA)

Stockport TPA provides early support for anyone vulnerable due to their health, wellbeing or situation, to prevent things getting worse and enable you to work towards independence.

Weight Management Service

A Better Life Weight Management Service is part of the Healthy Stockport family of services. The service encourages lifelong change by supporting clients in achieving a healthier lifestyle through group sessions and one-to-one support.

Exercise Referral Scheme

Life Leisure Stockport Physical Activity Referral (PARIS) exercise referral scheme is designed to help inactive people with chronic medical conditions increase their physical activity. You can self-refer via the Life Leisure website.

Coordination Services

START – Stockport Triage Assessment & Referral Team is part of the Healthy Stockport family and is the main point of access into health and wellbeing services in Stockport, including smoking cessation, weight management, physical activity, and alcohol support.

Stop Smoking

A Better Life Stop Smoking Service is part of the Healthy Stockport family. They provide free, local support starting with a chat with a friendly advisor to ensure they can offer the support you need.

Services include:

  • Personal quit plan tailored to your needs

  • Access to stop smoking treatments

  • Motivation and tips to stay on track

  • Help setting achievable goals to quit for good

DIABETES Research Opportunities

Help BEAT Diabetes is part of Research for the Future, an NHS campaign encouraging people in Stockport and across Greater Manchester to get involved in health research.

If you live with diabetes or have been told you are at risk of developing it, you can register to receive information about local research opportunities. There are lots of different types of research you can take part in, including questionnaires, joining discussion groups, or taking part in clinical trials.
