What is the NHS Type 2 Diabetes Path to Remission (T2DR) Programme?

A FREE programme to support you to:

  • Lose weight – approximately 2 stone (14kg) on average in 3 months

  • Stop or reduce your diabetes medications on Day 1 of the programme

  • Feel fitter, healthier and happier

  • Potentially put your diabetes into remission

NHS Type 2 Diabetes Path to Remission

Am I Eligible?

You need to be:

  • Registered with a Greater Manchester GP

  • Aged 18–65

  • Diagnosed with Type 2 diabetes within the last 6 years

  • Above a healthy weight with a BMI of 27 or higher (if you are from white ethnic backgrounds) or 25 or higher (if you are from Black, Asian or another ethnic background)

How is the Programme Delivered?

The programme has three phases over 12 months:

  1. Phase 1 (12 weeks): You'll follow a low-calorie diet – your choice of soups, shakes, and bars totalling 800–900 calories per day – all at no cost to you

  2. Phase 2 (6 weeks): You'll gradually replace these products with healthy, tasty meals

  3. Phase 3 (8 months): You'll receive ongoing support to help you maintain your weight loss

Our trained coaches will support you to lose weight and keep it off. You can choose to meet your coach either:

  • One-to-one in person

  • Through an app-based digital support
