NDH/CVD Reflecting on Your Health in 2025 and Setting Achievable Goals for 2026

As the year draws to a close, it’s a natural time to pause, reflect, and look ahead. Whatever health condition you have, taking stock of your health and wellbeing can help you understand what’s working, what’s been challenging, and what you’d like to focus on in the year ahead.
This isn’t about setting huge, overwhelming goals. It’s about small, meaningful steps that support better health, confidence, and quality of life.


1. Look Back Before You Look Forward


A few simple questions can help you reflect on 2025:

  • What went well for me this year?
  • What did I learn about my health condition and how my body responds?
  • Were there moments when I felt more in control, or less?
  • What habits supported my health? Which ones held me back?

Be honest but kind with yourself. Reflection isn’t about judgement, it’s about understanding, making some changes and moving on.


2. Set Small, Achievable Goals for 2026


Big resolutions often fail because they require big energy and big changes. Small goals, on the other hand, build success.
